That's not how most projects fail. Most people here have been on a software development project that reported progress every week, right up until the day it was canceled. There was never an assertion that the project would never work. You just needed someone senior enough in the organization to look at the project planning and realize it doesn't converge on completion in a relevant amount of time. Failure is seldom spectacular.
