Safety panel says NASA should have taken Starliner incident more seriously

I'd say the real problem is that Boeing never took their Starliner seriously.

During the incident, my sense was that top NASA brass had quietly written the Starliner off - but NASA had to halfheartedly go through all the motions, to appease political interests in Washington.
