A kind of trite complaint about how being ingenuine with people you trust or are close with does you both a disservice. You still have to be polite to people outside that trust boundary. I think a better approach would be to tell the asker how you're actually doing as an indirect challenge than "does it really matter?"

Perhaps explain that your well being is not important to you.
